Please complete this form each time you complete hours you would like to bank for our closure August 26th-30th. Remember this is an unpaid week off unless you accumulate time to be paid out that week. These hours must be approved by or offered by Lauren. Please add your name, the date you completed the hours, the amount of hours and the task/situation (for example approved deep clean, gave up lunch break… etc)
